RN - Registered Nurse - WOCN (Wound, Ostomy, Continence Nurse) - Med-Surg * Days - 40hrs/wk

General Summary:

The Registered Nurse - WOCN (Certified Wound, Ostomy, and Continence Nurse) is a professional care giver with responsibility and accountability for a group of patients for a designated time frame and provides care to these patients via therapeutic use of self, the nursing process, the environment / instrumentation, and other health care team members. The RN, WOCN assigns and directs the work of related staff.

Requirements:

  • Michigan Registered Professional Nurse licensure required.
  • Wound, Ostomy Nurse (CWON) certification required within one year of hire.
  • Bachelor of Science in Nursing preferred.
  • Current Basic Life Support (BLS/CPR) provider—required.

Essential Functions and Responsibilities:

  1. WOCN FUNCTIONS
    • Demonstrates competence and knowledge and serves as an expert resource in the assessment, treatment and care of patients with ostomies, wounds and incontinence care.
    • Provides direct care to patients with abdominal stomas, wounds, fistulas, drains, pressure ulcers and continence disorders and participates in assessment, planning, implementation and evaluation of patients with wounds, ostomy and continence care needs.
    • Provides ambulatory care services for the patient with a wound, ostomy or incontinence within the scope of WOC nursing practice.
    • Demonstrates effective communication with patients, families, team members and other key care groups
    • Partners with surgical specialties in providing care
    • Collaborates closely with Wound APP staff.
    • Provides support and education to patients, clinical staff and care givers for the patient
  2. NURSING PROCESS
    • Assess the patient's condition and physical, psychological, cognitive, social, rehabilitative, and discharge needs.
    • Develop nursing diagnosis utilized nursing judgment regarding health conditions and needs based on the nurse care plan.
    • Develop a plan of care using nursing diagnoses and setting patient outcome goals. Establish priorities of patient care based on essential patient needs and available resources of time, personnel, equipment and supplies.
    • Implement nursing plan of care and physician's orders. Use delegation skills and therapeutic use of self to coordinate and organize patient care delivery.
    • Evaluate the patient’s response to nursing interventions and privider ordered therapy / diagnostics. Update or modify plan of care to meet the current needs of the patient.
  3. PATIENT EXPERIENCE
    • Educate the patient and significant others based on the cognitive needs of each and defined by patient outcome goals.
    • Demonstrate knowledge of and respect for patient and significant others’ rights, including confidentiality.
    • Follow established standards in provision for safety.
  4. PROFESSIONALISM
    • Assume responsibility for own practice including appearance, attendance, punctuality and flexibility in scheduling.
    • Maintain a current knowledge base and use standards of care in professional practice.
    • Practice effective problem identification and resolution skills as a method of sound decision making.
    • Perform efficiently in emergency patient situations following established standards, remaining calm, informing appropriate persons, and documenting events.
    • Pursue self-directed learning experiences, continuing education, and seek direction when necessary.
    • Participate in and promote staff development activities and achievement of department goals.
  5. ORGANIZATIONAL STANDARDS
    • Participate in peer review, quality assurance, standards development, and research activities.
    • Demonstrate awareness of legal issues in all aspects of patient care and department function and strive to manage situations in a reduced risk manner.
  6. LEADERSHIP
    • Perform nursing procedures, assist physicians, and use equipment following established standards.
    • Delegate patient care needs to appropriate staff.
    • Directs and reviews the work of support staff to assure nursing care plan is fully implemented and patient needs are addressed.
  1. COMMUNICATION
    • Use written and oral communication through designated channels to attain desired patient outcomes as well as department and hospital goals.
    • Document concise, accurate and timely information in the patient care record.
  2. Performs other duties as assigned. These may include but are not limited to: Maintaining a current knowledge base of department processes, protocols and procedures, pursuing self-directed learning and continuing education opportunities, and participating on committees, task forces, and work groups as determined by management.
