  • Internal Medicine – Care Resources, Grand Rapids, MI – PA/NP

    University of Michigan Health-West is seeking a full-time, board eligible/board certified, Family Medicine or Internal Medicine Advanced Practice Provider (PA/NP) to assist in supporting Care Resources, an all-inclusive care program for the elderly (PACE). High caliber, quality focused, innovative providers should apply to join our team in effort to continue to expand and provide care to local communities throughout West Michigan.

    The PACE Program embraces the physical, mental, and social health in a “one-stop shopping” model. The model has an eleven-discipline model that includes primary care, home care, rehabilitation, prescription medicine, day center & clinic, medical specialists, nutrition services, transportation, and social services with the participant at the center of these services. The position is a critical part of the Care Resources program and will see participants of the program.

    PACE is a Medicare and Medicaid funded option that gives community-based care and services to people 55 or older, helping to prevent nursing home placement. PACE provides coverage for prescription drugs, doctor care, transportation, home care, checkups, hospital visits, and nursing home stays when necessary. Private pay is also available.

    Candidate will need to coordinate and collaborate with a multidisciplinary team to ensure a positive experience for all patients, families, and staff.

Provider will be performing full spectrum of outpatient primary care services for patients, including primary, acute, chronic, and specialty care to patients of all ages and backgrounds.

  • Must have experience working in Geriatrics, Internal Medicine, or Palliative Care
  • Provider exercises autonomy in medical decision making and provides a broad range of diagnostic and therapeutic services to adult patients. This may include managing patients with complex, acute, critical, and chronic health conditions
  • Assists patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies.
  • Assesses the patient’s ability to navigate the increasingly complex healthcare continuum, assists in identifying potential barriers to timely and appropriate health care, and develops plan of care to overcome such barriers
  • The individual collects and assists in the interpretation of reports from diagnostic and laboratory tests, radiographic procedures, and clinical studies. The position typically analyzes, compares, and evaluates various courses of action and has the authority to make independent decisions on matters of significance, free from immediate direction, within the scope of their responsibilities.

    Qualifications:

  • Licensed as Registered Nurse. Certified and licensed as a nurse practitioner by the State of Michigan.
  • Certified and licensed as a physician assistant by the State of Michigan
  • Graduate from an accredited training program
  • BLS certification or acquisition within 90 days of employment. BLS must be maintained current throughout time of employment at UMH-West.
  • Any additional requirements or skills specific to service line or site
  • Must have experience working in Geriatrics, Internal Medicine, or Palliative Care
